Terms and Conditions: Alumni who apply to and are accepted as mentors in the PLAN Connector Program must read the information below and agree to the terms listed.
CONFIDENTIALITY
Your email address will be provided to mentees once the match is confirmed. All information you hear or obtain from the mentee must be kept confidential by you indefinitely unless disclosure is permitted by the student or is required by law.
CANCELLATIONS
If an upcoming meeting needs to be rescheduled or cancelled, mentors are responsible for contacting the mentee directly. Mentors are required to contact the program administrator (MYPAC) if they wish to pause or cancel their participation in the PLAN Connector Program.

QUALITY OF OUTCOME
I acknowledge and understand that although student participants complete a detailed application, York University assumes no responsibility over the quality or outcome of my participation in the PLAN Connector Program
PROMOTIONAL CONSENT TO YORK UNIVERSITY
I hereby grant permission to York University and its representatives the right to reproduce, use, exhibit, display, broadcast and distribute the uploaded images and recordings in any media now known or later developed for promoting, publicizing or explaining York University and its activities and for administrative, educational or research purposes. I hereby grant permission to York University to use my name with these images and recordings.
PROMOTIONAL CONSENT TO MYPAC
I hereby grant permission to MYPAC the right to reproduce, use, exhibit, display, broadcast and distribute the uploaded images and recordings in any media now known or later developed for promoting, publicizing or explaining MYPAC and its activities and for administrative, educational or research purposes. I hereby grant permission to MYPAC to use my name with these images and recordings.
